Patient Identification (PID) segment — HL7v2 v2.5.1, with v2.7+ extensions.

Primary patient demographics segment. 39 fields per HL7 v2.5.1 specification,
plus v2.7+ optional field 40 (patient_telecommunication_information), which
replaces the deprecated PID-13/PID-14 in v2.7.
